The Smiles Club is a program designed for Premier Dental Care patients who DO NOT have dental insurance.
The Smile Club member can choose membership type:
$399 | Individual Annual Membership
$749 | Individual + Spouse Annual Membership
$319 | Additional Family Members Annual Membership
The Smile Club member receives the following benefits:
- Two cleanings in a 12-month period
- Two examinations by the doctor in a 12-month period
- One set of x-rays in a 12-month period
- One fluoride treatment in a 12-month period
- 20% courtesy on all additional dental treatment at our office for the 12-month period
The Smiles Club membership is renewable on a an annual basis. The price is subject to change at renewal. Membership is non-refundable and all benefits are usable in the 12-month period after membership is paid in full. There are no exceptions to this policy. The Smiles Club entitles members to a generous 20% courtesy: therefore, we require that all treatment be paid in full at the time of the appointment. If using CareCredit to extend payments, discounts will be adjusted to 10% to account for our finance charges. This plan may not be combined with any other offers, other plans or insurance submission. The plan will not apply for any outside referrals. The courtesy and The Smiles Club membership benefits cannot be combined with financing options such as Springstone or other discount promotions.
